Pharmacological action

Peripheral vasodilator with a primary effect on the venous vessels, antianginal drug.

The effects of nitroglycerin due to its ability to release a molecule of nitric oxide, which is a natural endothelial relaxing factor. Relaxation of smooth muscle of the vascular wall causes vasodilation, reducing venous return to the heart (preload) and resistance to systemic circulation (afterload). This reduces the work of the heart and myocardial oxygen demand. Expansion of coronary vessels improves coronary blood flow and promotes its redistribution in reduced blood supply, increasing oxygen delivery to the myocardium. Reducing venous return results in lower filling pressure, improve the blood supply to the subendocardial layers, reduce the pressure in the pulmonary circulation and the regression of symptoms with pulmonary edema. Nitroglycerin has a central inhibiting effect on sympathetic vascular tone, inhibiting the formation of vascular component of pain. Nitroglycerin also relaxes the smooth muscle cells of the bronchi, urinary tract, gallbladder, bile duct, esophagus, small and large intestines, and their sphincters.

Dosage regimen

The drug is used sublingually. При каждом нажатии на дозирующий насос высвобождается 1 dose (400 mcg nitroglycerin) aerosol.

To relief of angina attack при первых его признаках 400-800 g (1-2 dose) наносят на или под язык на фоне задержки дыхания. Интервал между повторным введением дозы должен быть не менее 30 sec. При необходимости препарат вводят повторно с 5-минутными интервалами, but not more 3 doses within 15 m.

Перед употреблением упаковку не следует встряхивать, держать при распылении нужно вертикально, прямо перед собой. После каждого распыления аэрозоля следует закрывать рот, рекомендуется сразу не глотать.

Side effect

CNS: blurred vision, headache (especially at the beginning of treatment, при продолжении терапии – уменьшается), dizziness, weakness, restlessness.

Cardio-vascular system: hypotension (incl. orthostatic), reflex tachycardia, flushing of the face; rarely (при резком снижении АД) – усиление симптомов стенокардии (парадоксальные “нитратные” reaction).

From the digestive system: dry mouth, nausea, vomiting.

Dermatological reactions: dermatitis, skin rash.

Contraindications

- Shock;

- Vascular collapse;

— острый инфаркт миокарда с гипотензией;

— констриктивный перикардит;

— закрытоугольная форма глаукомы;

Cautions

В связи с повышенным риском развития ортостатических реакций в период применения препарата следует соблюдать осторожность при резком переходе в положение стоя из положения лежа или сидя, при длительном стоянии, выполнении физических упражнений, при жаркой погоде.

Отмену препарата следует производить постепенно и под контролем врача.

Overdose

Drug Interactions

Другие антигипертензивные препараты, antidepressants, ингибиторы ФДЭ (sildenafil), этанол могут усиливать гипотензивный эффект Нитроспрея. При этом эффективность Нитроспрея может снижаться.

При одновременном применении с Нитроспреем возможно снижение антикоагулянтного действия гепарина.
