Characteristic.
1,4-dihydropyridine derivatives - calcium antagonist II generation.
Pharmacological action.
Gipotenzivnoe, vasodilator, antianginalnoe, nephroprotective.
Application.
Arterial hypertension (monotherapy or in combination with beta-blockers and diuretics), diabeticheskaya nephropathy (in the presence of contraindications to or intolerance to ACE inhibitors).
Contraindications.
Severe heart failure, sick sinus syndrome, aortic stenosis, severe hypotension, The first week after myocardial infarction, pregnancy, lactation.
Pregnancy and breast-feeding.
Contraindicated in pregnancy. At the time of treatment should stop breastfeeding.
Side effects.
Cardio-vascular system and blood (hematopoiesis, hemostasis): flushing of the face and upper torso, gipotenziya, tachycardia, anemia, thrombocytopenia.
From the nervous system and sensory organs: headache, drowsiness, paresthesia; rarely visual impairment, myalgia, tremor (at higher doses).
With the genitourinary system: peripheral edema (swelling of the ankles, lower limb); rarely, an increase in the frequency of urination; worsening of renal function (in renal failure).
From the digestive tract: nausea, a feeling of fullness, vomiting, constipation; seldom-diarrhoea; in some cases, intrahepatic cholestasis.
For the skin: эritema, prurit, hives, maculopapular rash.
Other: withdrawal (5–10%); rarely giperplazia gums, transaminase elevation, gynecomastia (in elderly patients).
Cooperation.
Antihypertensive, blockers H2-enhance the effects of histamine receptor, Estrogens, NPVS-weakened. May increase the plasma concentration of cardiac glycosides with the development of toxic effects. Metoprolol, acebutolol, atenolol and ranitidine may reduce the plasma clearance.
Dosing and Administration.
Inside, usually 20 mg in the morning. Depending on the effect and endurance daily dose can be gradually increased to a maximum of- 40 mg (by 20 mg 2 once a day) or reduce to 10 mg.
Precautions.
Be wary of during the drivers of vehicles and people, skills relate to the high concentration of attention. During treatment should refrain from drinking alcohol.
