Pharmacological action
The preparation with antimicrobial and Valium action for local application in an ENT-practice.
Chlorhexidine has a broad antibacterial spectrum of action against gram-positive and gram-negative bacteria. It acts by destroying the cytoplasmic membrane of the bacterial cell. Ineffective against the yeast fungi, dermatofitov, mykobakteryy, Some species Pseudomonas spp. and Proteus spp.
Chlorhexidine is most effective in the neutral or weakly alkaline medium. In the acidic environment of its activity is reduced. Efficiency is reduced in the presence of soaps, blood and purulent discharge.
Application lozenge chlorhexidine results in a significant reduction of the content of bacteria in saliva. The use of chlorhexidine for a few months, accompanied by a decrease of its efficiency by reducing the sensitivity of bacteria.
Benzocaine - mestnый anesthetic, quickly and for a long time, it relieves pain in the mouth and throat. Benzocaine penetrate the lipophilic areas of the cell membrane and acts on the peripheral pain receptors of the skin and mucosa. Benzocaine anesthetic effect comes through 15-30 sec; as diluting agent saliva anesthetic effect gradually decreases (during 5-10 m).
Pharmacokinetics
No clinical data chlorhexidine suction through the oral mucosa human. Chlorhexidine may be stored in the saliva to 8 no.
Benzocaine slightly soluble in water, which leads to its slow absorption. How all ester derivatives of PABA, cleaved by esterases in plasma and liver. As a result, formed and PABA ethyl alcohol, A metabolyzyruyuschyysya to atsetylkoэnzyma. PABA is subject to conjugation with glycine and excreted by the kidneys unchanged.
Testimony
Infectious-inflammatory diseases of the mouth or throat:
- Pharyngitis, sore throat and other inflammatory diseases of the throat;
- Stomatitis;
- Gingivitis.
Dosage regimen
The tablets should be slowly dissolve in the mouth until dissolved.
The drug should be used to start immediately after the onset of symptoms and continue to receive within a few days after the disappearance of symptoms.
Adults and older children 12 years appoint 1 tab. every 1-2 hours as needed, but not more 8 tab. / day.
For children 4-12 years to appoint 4 tab. / day.
Side effect
Prolonged use may be a short-term disturbance of taste and numbness of the tongue; it is also possible reversible discoloration of the teeth and tongue.
If you have a hypersensitivity to benzocaine in rare cases may develop hypersensitivity reactions up to anaphylactic shock.
Benzocaine can cause the development of methemoglobinemia in children.
Contraindications
- Wound and ulcerative lesions of the mouth or throat;
- Low concentrations of cholinesterase in the blood plasma;
- Children up to age 4 years;
- Hypersensitivity to any component of the drug.
FROM caution and under the supervision of a physician should be prescribed in patients with erosive, desquamative changes in the oral mucosa.
Pregnancy and lactation
Lack of experience of the drug in pregnant and lactating women.
Use of the drug Geksoral® Tubbs during pregnancy and lactation is possible only in cases, the expected benefit to the mother outweighs the potential risk of adverse effects to the fetus and child.
Cautions
Patients with diabetes should be informed, that carbohydrate content 1 tab. matches 0.1 HE.
Overdose
When used properly, an overdose of the drug can not be, tk. chlorhexidine practically not absorbed into the blood, and the amount of intake benzocaine very little.
Chlorhexidine
Chlorhexidine hydrochloride is poorly soluble in water, all cases of overdose described only upon receipt of highly water-soluble chlorhexidine gluconate. In these cases it was marked by damage to the mucous membrane in direct contact with chlorhexidine gluconate, and the systematic concentration reversible increase of liver enzymes. Specific treatment is not available.
Benzokain
Overdose is possible only if used incorrectly.
Symptoms: possible toxic effects on the central nervous system, manifested first tremor, vomiting, cramps, and later - CNS depression. Due to respiratory depression may develop coma. High concentrations of toxic cause bradycardia, blockade AV-conduction and cardiac arrest. Benzocaine can cause methemoglobinemia, especially in children, accompanied by dyspnea and cyanosis.
Treatment: necessary cause vomiting and stomach wash. Perhaps the appointment of activated carbon. When hypoxia and anoxia is recommended mechanical ventilation with oxygen support, cardiac arrest - cardiac massage. In convulsions prescribed diazepam or barbiturates fast (contraindicated in anoxic seizures); in severe cases, after intubation and respirator use suxamethonium chloride. Circulation should support the introduction of blood plasma or electrolyte solutions. When methemoglobinemia can be used to 50 ml 1% solution of methylene blue / in.
Drug Interactions
Benzocaine due to the formation of its metabolite 4-amino-benzoic acid reduces the antibacterial activity of sulfonamides and aminosalicylates.
Sucrose, polysorbate 80, insoluble magnesium salts, zinc and calcium reduce the effect of chlorhexidine.
